Care of the Community, by the Carers, for the Community

Launching subject to CQC registration

Reablement

Regaining confidence and independence after illness or hospital.

Coming home from hospital can feel daunting. Our reablement service provides short, intensive, goal-focused support — typically up to six weeks — to help people relearn daily skills and regain their independence.

We work alongside NHS teams, discharge coordinators, physiotherapists and occupational therapists, keeping every plan joined-up and focused on what matters to the individual.
